Job Summary
Responsible for providing professional nursing care to Hospice Services patients, identifying patient/family needs and providing supportive care in accordance with physician's orders and plan of care.
Assumes primary responsibility for a patient/family caseload, including assessment, planning, implementation, and evaluation of the nursing process, and collaborates with the Interdisciplinary Group.
Provides Hospice Services nursing care based on systematic assessment focusing on pain control and symptom management, and teaches, supervises, and counsels patients/families regarding physical care and problems related to the terminal condition.
