Job Summary

  • The RN Care Manager provides longitudinal care management services, by phone and face to face, to promote wellness, improved outcomes and self-care within a defined population of patients with chronic condition(s), complex health needs, and support their appropriate utilization of health services.
  • Conducts transitional care outreach to patients within 48 hours of a hospital discharge and assists with effective coordination of services internally and externally.
