Kern River Transitional Care is seeking a Remote Social Worker responsible for planning and implementing social service programs to address the emotional and social needs of residents. The ideal candidate will have a Bachelor’s Degree in Social Work or a related human services field, along with supervised experience in a healthcare setting

Job Summary

  • This role is responsible in assisting in planning, developing, organizing, implementing, evaluating, and directing our facility’s social service programs to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
  • Assist residents in achieving the highest practicable level of self care, independence and well being, and provide medically related social services so that the highest practicable physical, mental and psychosocial well being of each resident is attained or maintained.
  • Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident and assist in making outpatient appointments and discharge planning.
