Job Summary
Responsible for coordinating the care and services, partnering with the oncologist, advocating for the patient and facilitating timely access to managing hematologic and oncologic care through the cancer care continuum.
Triage clinical phone calls from oncology/hematology patients using clinical critical thinking and communicates with physician/provider to help resolve patient issues.
Works with patients to ensure the ability to obtain their medications, educate patient on medication and check on adherence.
