Job Summary

  • The CNA – Hospice role support patients and families with dignity, empathy, and professionalism, ensuring comfort and quality of life during a deeply meaningful stage of care.
  • Provide personal care, hygiene support, exercise assistance, and patient ambulation in accordance with state and federal regulations.
  • Observe, document, and promptly report changes in the patient’s physical or mental condition to the RN Case Manager.
