Job Summary

  • Assists patients and families with complex medical and behavioral health needs to navigate uneventfully, efficiently, and as effectively as possible through the course of their illness to improve or maintain social, emotional, functional and physical health.
  • Provides care coordination services to patients and their families as a member of an interdisciplinary team, managing transitions and monitoring follow-up across the continuum of care.
  • A $2,000 Sign-On Bonus is offered for new-to-NCH employees for this full-time, Monday - Friday position.
