Job Summary
The Community Health Worker (CHW) will provide education, resources, and health education to patients, local community-based organizations, and community members.
This position requires collaboration with various team members on multiple projects aimed at addressing public health responses and providing culturally appropriate outreach to hard-to-reach populations.

Key Requirements
High School diploma/GED
Two (2) years of general experience (Level I)
Four (4) years of experience as a CHW (Level II)
Nine (9) years of experience as a CHW (Level III)
Must enroll in a certified CHW Training Program within 45 days of employment (Level I)
Completed certified CHW training program (Level II)
CHW certification from ARCHWA (Level III)
