Job Summary
This role leads the implementation and ongoing oversight of the Care Coordination program for MSHO and MSC+ Government Programs, ensuring compliance with DHS contractual requirements.
The position partners closely with internal stakeholders and external partners to ensure changes are implemented effectively, on time, and in full regulatory compliance.
The Manager provides direct leadership and oversight of the GP Partner Relations team, ensuring clear accountability, staff development, and sustained compliance.
