  • The patient population served by the advanced practice provider will be post heart transplant patients, triaging patients and practicing in both inpatient and outpatient settings requiring complex monitoring and therapies.
  • Practices with a variety of specialty-based populations, functions in collaboration with a physician according to a standard care arrangement and specialty certification.
