Job Summary
Develops and oversees nursing care plans that individually address each patient’s needs, caring for the patient’s mind, body and spirit while supporting their families and caregivers.
Makes initial nursing evaluation visits, collaborates with patients and families, and assesses, collects, and analyzes comprehensive data to determine nursing diagnoses and plan of care.
Supervises clinical care provided by LPNs and Home Health Aides and serves in an on-call rotation providing patient care outside of regular business hours.
