Provides medically related social services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident, including emotional support and assistance with health care decisions.
Assists residents with personal shopping, inventory tracking, and securing resources from community and social services agencies, while developing strong working relationships with internal departments and external agencies.
