Job Summary
Habitat Health provides personalized, coordinated clinical and social care as well as health plan coverage through the Program of All-Inclusive Care for the Elderly (“PACE”).
In this role, you will assist with the provision of direct participant care by performing routine care procedures and technical/administrative support services in the clinic.
You will partner closely with the interdisciplinary team to share any feedback on the participant’s well-being or changes in status.
