Job Summary
This role is responsible for assisting in the planning, developing, and directing of social service programs to meet the medically related emotional and social needs of residents.
The incumbent must assist residents in achieving independence while providing consultation to staff and community agencies to solve complex social and medical problems.
Employees are required to maintain strict confidentiality of all resident care information including protected health information and report any suspected fraud or unauthorized disclosures.

Key Requirements
Bachelor's Degree in Social Work (BSW) or related human services field
Minimum one year supervised social work experience in healthcare setting
State social work licensure (LSW, LMSW, LCSW) if required by law
Completion of mandatory in-service trainings including HIPAA and abuse prevention
