Salary
$71,100 - $97,800 per year; Bonus incentive plan; Medical, dental, vision, 401(k), time off

Key Requirements
Active Registered Nurse (RN) license in Virginia
Two (2) years of experience in health care/case management
One (1) year of experience with Cardinal Care Priority Population criteria
Knowledge of community health and social service agencies
Intermediate to advanced computer skills
Ability to work a full-time schedule
Valid state driver's license and personal vehicle liability insurance
