Job Summary
This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care.
Coordinate care for long-term care residents using Ennoble Care’s EMR, maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions.
Ennoble Care offers a variety of programs including, remote patient monitoring, behavioral health management, and chronic care management, to ensure that our patients receive the highest quality of care.

Key Requirements
Minimum 1 year patient-facing experience
Active clinical certification or licensure
