Job Summary
Health Navigators will be assigned to specific employer clients and will manage the employee population specifically in their healthcare needs.
The Health Navigator will assist employees with support such as locating in-network providers, identifying appropriate levels of care, managing chronic conditions, preventative care plans, healthy lifestyle habits, education resource management, and maximizing their benefit offerings.
The Health Navigator will engage in proactive outreach to client employee population and provide broad engagement events and activities to promote program participation.

Key Requirements
Associates Degree of Nursing required
