This role provides oversight on facilities' weekly PDPM meetings and monitors effective utilization of PDPM tools for skilled patients.
The incumbent is responsible for assessing health status, ensuring accurate MDS completion, and coordinating interdisciplinary participation in care plans.
Candidates must maintain confidentiality of resident care information while supervising staff and managing level of care changes.

Must-have
Accredited School of Nursing graduation
State Board of Nursing good standing
PDPM tools and MDS assessment completion
Interdisciplinary care plan coordination
Federal and state regulatory compliance
Nice-to-have
Bachelor of Science Degree in Nursing preferred
3 years supervisory experience preferred
Long Term Care experience preferred
PCC software knowledge preferred
Microsoft Suite proficiency required
