Job Summary
RN Case Managers work collaboratively with inter-professional teams across the continuum of care to facilitate and ensure effective transitional care coordination.
This is a PRN role, requiring 2 Mondays, 2 Fridays, and 1 weekend shift per month, with floating across Cleveland Clinic Main Campus required.
The ideal caregiver will ensure compliance with standards of accrediting bodies, state mandated revised code, and CMS Conditions of Participation.

Key Requirements
Graduate from an accredited school of professional nursing
Current state licensure as a Registered Nurse (RN)
Basic Life Support (BLS) Certification
3 years of recent RN clinical experience
Bachelor of science in nursing (BSN) preferred
Certification by an approved entity within one year of eligibility preferred
2 years of recent experience in utilization review, quality or care management preferred
