Job Summary
The RN Care Manager provides longitudinal care management services, by phone and face to face, to promote wellness, improved outcomes and self-care within a defined population of patients with chronic condition(s), complex health needs, and support their appropriate utilization of health services.
Conducts transitional care outreach to patients within 48 hours of a hospital discharge and assists with effective coordination of services internally and externally.

Key Requirements
Baccalaureate Degree, ADN, or Diploma RN Degree
3 years RN experience
current BLS certification
Valid Arkansas Registered Nursing License
Certification in Case Management (e.g. CCM, ANCC) or obtain within 1 year of hire date
BSN preferred
Care Management experience in an Ambulatory setting
