Job Summary

  • The primary purpose of this job position is to assist in planning, developing, organizing, implementing, evaluating, and directing our facility’s social service programs to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
  • Assist residents in achieving the highest practicable level of self-care, independence and well-being and provide medically related social services so that the highest practicable physical, mental and psychosocial well-being of each resident is attained or maintained.
