Key Requirements

  • Doctorate of Pharmacy (PharmD) degree
  • KY or IN Board of Pharmacy license

  • PGY-2 residency OR 3+ years experience + BCOP
  • 5+ years experience + BCOP alternative path
  • Current BLS and ACLS certification
