Assists in providing direct care such as ambulation, feeding, bathing and grooming.

Experience with dementia and palliative care

Key Requirements
Completed a PSW Program or HCA/CCA certification
Experience in Long Term Care or community home care setting
Knowledge and experience in care planning systems and documentation
Experience working independently in Community Home Care is an asset
Access to reliable vehicle in community home care is required
