Job Summary
The Inpatient Care Navigator will complete comprehensive psychosocial assessments focusing on patient needs for transition to the next level of care and collaborate with physicians, nurses, and other disciplines for coordinated discharge planning.
Responsibilities include identifying and navigating patient testing and treatment to reduce discharge barriers, communicating with Utilization Review staff, identifying and connecting patients with community resources, and documenting assessments and interventions.
The role offers variable part-time employment with every other weekend commitment, day shift, and benefits such as work-life balance, recognition programs, a 403(b) retirement plan, and EAP support.

Key Requirements
BSW or MSW or AS/BS Nursing degree
3 years SW experience in diverse clinical settings
LBSW, LMSW, LCSW required for SW
1-3 years RN experience in diverse clinical settings
RN licensed in Maryland
American Heart Association BLS certification
Prior case management or discharge planning experience for RN
