Job Summary

  • This role is responsible for assisting in the planning, developing, organizing, implementing, evaluating, and directing of the facility's social service programs to meet resident needs.
  • The incumbent must assist residents in achieving the highest practicable level of self-care, independence, and well-being while addressing emotional problems related to illness and admission.
  • The position requires maintaining confidentiality of all resident care information including protected health information and reporting suspected incidents of fraud or unauthorized disclosure.

Key Requirements

  • Prior long-term care or skilled nursing facility experience preferred
