Job Summary
This role is responsible for assisting in the planning, developing, organizing, implementing, evaluating, and directing of the facility's social service programs to meet resident needs.
The incumbent must assist residents in achieving the highest practicable level of self-care, independence, and well-being while addressing emotional problems related to illness and admission.
The position requires maintaining confidentiality of all resident care information including protected health information and reporting suspected incidents of fraud or unauthorized disclosure.

Key Requirements
Bachelor's Degree in Social Work (BSW)
One year supervised social work experience in healthcare setting
State social work licensure (LSW, LMSW, LCSW) if required
Completion of mandatory in-service trainings including HIPAA
Prior long-term care or skilled nursing facility experience preferred
