Job Summary

  • Completes psychosocial assessments for patients' post-hospital care and assists them in coping with stress related to hospitalization.
  • Works with the Clinical Care Management team to assess and evaluate patients for appropriate care levels and serves as a resource for hospital staff regarding psychosocial components of illness.
  • Provides information and education on community resources and options for post-hospital care, making referrals as needed and reporting suspected cases of abuse/neglect/exploitation.
