Centerville Post Acute is seeking a Full-Time MDS Assistant (RN) to oversee and coordinate the Minimum Data Set (MDS) assessments for skilled patients in a remote capacity. The ideal candidate should have a Bachelor of Science Degree in Nursing, supervisory experience, and proficiency in MDS processes and tools, alongside a strong ability to communicate and collaborate with interdisciplinary teams

Job Summary

  • Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks facility effective utilization of PDPM tools.
  • Assesses and determines the health status and level of care of all new admissions, ensuring accurate and timely completion of all MDS Assessments.
  • Responsible for all level of care changes within the facility, notifying all departments and transmitting appropriate forms to the appropriate agency for processing.
