Job Summary

  • The primary purpose of this position is to oversee the facility to ensure assessments are completed per guidelines.
  • The role requires tracking and monitoring facility effective utilization of PDPM tools including Physician Diagnosis Verification and Weekly CSR.
  • Candidates must ensure the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant change.

  • 3 years of supervisory experience preferred
  • Long Term Care Experience Preferred
  • Proficient in Microsoft Suite products
  • Good standing with State Board of Nursing
