Job Summary

  • Supports new parents in providing newborn care, offers physical assessments, addresses questions, identifies needs, and helps to identify and facilitate referrals to community services or resources.
  • Provides home visits in the determined catchment area and works in a multidisciplinary environment to schedule, visit, and support families.
  • Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses and maintains a current Ohio Nursing License.

Key Requirements

  • BLS certification

  • One year of experience in child/maternal health, social work, or related field
  • Experience working with diverse populations
  • Valid Ohio driver's license and proof of auto insurance
