Job Summary

  • Assists patients and families with complex medical and behavioral health needs to navigate uneventfully, efficiently, and as effectively as possible through the course of their illness to improve or maintain social, emotional, functional and physical health.
  • Provides care coordination services to patients and their families as a member of an interdisciplinary team, facilitating communication and collaboration amongst the healthcare team.
  • Develops patient’s/family self-management skills through education and resource provision and manages transitions between settings, caregivers and providers.
