Key Requirements

  • Graduation from accredited medical school
  • Successful ACGME fellowship in cardiology or cardiothoracic surgery
  • Valid Oregon license as MD or DO
  • Board certification in cardiology, interventional cardiology, advanced heart failure, or cardiothoracic surgery
  • Minimum 10 years in cardiology with 5 years in leadership
  • Maintain unrestricted DEA number and Medicare/Medicaid participation ability
