Job Summary

• Assesses patients to determine their discharge planning and/or post-acute transition needs, developing and implementing discharge plans utilizing internal and external resources for safe transitions.
• Works collaboratively with physicians and the multidisciplinary team to ensure communication and exchange of input related to patient's specific care needs, utilizing clinical judgment and evidence-based guidelines.
• Advocates in the patient's best interest for necessary funding, treatment alternatives, timelines, and coordination of care, while identifying community and caregiver resources for continuity of care.

• Minimum three years of acute care experience
