Essential duties include participating in planning and conducting individual and group activities, communicating with employees, residents, and families, and assisting in developing monthly activity calendars and maintaining attendance records.
The role involves assisting with discharge planning, care plans, resident assessments, and arranging transportation for residents when necessary.
