Job Summary
As an independent contractor, or direct hire, the Nurse Practitioner will provide comprehensive primary care to residents as part of the interdisciplinary health care team.
The Nurse Practitioner will manage the care of residents by providing pharmacological, complementary and/or counseling interventions, and provide outreach and transitional services to LTCH residents who return to the community.
