Job Summary
Blended Case Management (BCM) assists children and adults with behavioral health/medical needs by building a strong network of community resources and services.
A Blended Case Manager will advocate for his/her client, help negotiate complex systems, assist in maintaining and finding access to basic living needs, and provide crisis intervention.

Key Requirements
Bachelor’s degree in human services related field OR Associates Degree with 12 College Credits
Two (2) years’ experience in mental health/human service field
