Lakeport Post Acute is seeking a Remote RN MDS Coordinator who will be responsible for administering patient assessments, coordinating care plans, and overseeing nursing staff within a long-term care facility. The position requires a licensed LPN/LVN with strong organizational and leadership skills, as well as knowledge of Medicare and Medicaid regulations
Job Summary
Administer patient assessments, oversee the assessment process, setting the assessment schedules and assuring that assessments are done in an accurate and timely manner.
Prepare and administer medications as ordered by the physician.
Participate in developing, planning, conducting, and scheduling in service training classes that provide instructions on “how to do the job,” and ensure a well educated nursing service department.
