This role provides oversight on facilities' weekly PDPM meetings for skilled patients and tracks effective utilization of PDPM tools.
The incumbent is responsible for assessing new admissions, ensuring accurate MDS completion, and coordinating interdisciplinary participation in care plans.
The position requires maintaining confidentiality of resident information while supervising staff and managing level of care changes.
