The primary purpose of this position is to provide each assigned resident with routine daily care and services in accordance with their assessment and care plan.
Employees must ensure correct resident information when assisting with ALDS, meals, and other tasks while maintaining strict confidentiality of protected health information.
The role requires frequent checking of residents unable to call for help, assisting with personal care needs, and reporting all accidents or incidents immediately.

Skills & Requirements
Must-have
Assist residents with daily living activities
Monitor and report changes in resident condition
Maintain confidentiality of protected health information
Perform lifting and moving of residents up to 25 pounds
Document care entries in PCC system and incident reports
Nice-to-have
Create warm and positive atmosphere for residents
Cooperate effectively with inter-departmental personnel
Demonstrate ability to solve practical problems
Key Requirements
High school education or equivalent required
Proficiency in Microsoft Suite products required
Training on specific care tasks and equipment usage required
