Provides support for individuals and/or groups of persons with disabilities in the community, assisting them in identifying and communicating their needs and implementing services to address their goals.
Duties include educating on rights, promoting healthy living, teaching independent living skills, providing transportation, assisting with personal care, and documenting care.
