Job Summary

  • This position is responsible to assist patients referred from Psychiatry, Emergency Department, and other units located within the facility in achieving their discharge goals for follow up treatment.
  • The Behavioral Health Patient Navigator is instrumental in educating patients on how to access available mental health resources and services within their community.
