Job Summary
As a Community Health Worker, you will work with our Infant and Maternal Health population and play a vital role in connecting diverse and underserved populations to health and social service systems.
You will strengthen individual and community capacity through outreach, education, informal counseling, social support, and advocacy, with a particular emphasis on helping patients and community members access resources related to food insecurity, housing, healthcare, utilities assistance, and transportation.
A caregiver who excels in this role will establish trusting relationships with patients and their families while providing support, encouragement and feedback.
