Job Summary

• Under the direction of the collaborating physician(s), provides care to patients, obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results.
• Prescribes pharmacologic and non-pharmacologic therapies as appropriate, demonstrates evidence-based practice, and participates in the development, implementation, and evaluation of standards of practice in the specialty.

• Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL
• Federal DEA and Illinois Controlled Substance License
• Completion of master’s level program
• BLS/CPR certification
