** Jefferson County Human Services is seeking a Family Support Caseworker to provide case management and support services to individuals with disabilities and older adults, focusing on accessing necessary resources and programs. The role requires extensive knowledge of community resources and the ability to advocate effectively for clients while managing a high volume of cases. **

Job Summary

  • This role is responsible for member-facing case management, including receiving and processing HCBS referrals for IDD, Non-IDD, and FSSP Waivers and programs, conducting eligibility screen tools and assessments, determining developmental delays or disabilities, working with our LTC Financial Program Partners to establish and confirm financial eligibility, connecting members to community resources, developing FSSP Service Plans, and monitoring FSSP Funding needs.
