This role provides oversight on weekly PDPM meetings and monitors facility utilization of PDPM tools to ensure optimum outcomes.
The incumbent is responsible for assessing new admissions, ensuring accurate MDS completion within regulatory timeframes, and coordinating interdisciplinary care conferences.
Candidates must maintain confidentiality of resident information while supervising staff and managing level of care changes across the facility.

State Board of Nursing good standing

3 years supervisory experience preferred
Long term care experience preferred
PCC software knowledge
Microsoft Suite proficiency
