  • Provide professional nursing care under the direction of a RN Case Manager and/or the Clinical Manager, performing skilled nursing visits for patients as directed by the plan of care.
  • Recognize and report changes in patient condition, communicate information on a timely basis, and provide care within the scope of clinical practice including assessment, medication monitoring, patient/family teaching, and emergency intervention.

Key Requirements

  • Graduate of approved, accredited school of professional nursing
  • LPN license in Massachusetts
  • American Heart Association BLS certification
  • One year home care/hospice experience preferred
  • Knowledge of homecare regulatory/reimbursement
  • Good written/verbal communication skills
  • Strong organizational/time management skills
  • Valid driver's license, good driving record, reliable transportation
