Job Summary

  • The primary purpose of your job position is to assist in planning, developing, organizing, implementing, evaluating, and directing our facility’s social service programs in accordance with federal, state, and local standards to meet residents' emotional and social needs.
  • You will assist residents in achieving the highest practicable level of self-care, independence, and well-being while coordinating with various departments and community agencies.
  • This role involves maintaining confidentiality of resident information, reporting suspected fraud, and supporting discharge planning and outpatient appointments.
