The primary purpose of this position is to provide each assigned resident with routine daily care and services in accordance with their assessment and care plan.
Employees must report all changes in resident condition, accidents, incidents, and suspected abuse to the Health and Wellness Director or Executive Director immediately.
This on-call role requires proficiency in Microsoft Suite products and the ability to perform physical tasks such as lifting up to 25 pounds.

Skills & Requirements
Must-have
Assist residents with daily living activities
Monitor and report changes in resident condition
Maintain confidentiality of protected health information
Perform lifting and moving of residents
Document care entries in PCC system

Create warm and positive atmosphere for residents
Cooperate with inter-departmental personnel
Participate in regular nursing reports
Handle end-of-life and post-mortem care
Assist with restorative procedures
Key Requirements
High school education or equivalent
Proficiency in Microsoft Suite products
Training in specific care tasks performed
Compliance with blood/body fluid exposure policies
