Key Requirements

  • Current state license as Physician Assistant or APRN
  • Credentialing certification as PA or Nurse Practitioner
  • Eligible for State Controlled Substance License II-V
  • Eligible for Federal DEA Certificate II-V
  • BLS Certification for Healthcare Providers
