Job Summary
The Discharge Planner will offer clinical assessment of social, emotional, medical, spiritual and physical needs, assessing the patient holistically and developing a plan of care.
This role will be actively involved with patient assessments and interviews, communication with family members, and contributing to a safe and timely discharge of patients requiring post-acute care services.
