Skills & Requirements
Must-have
LVN or LPN valid license required
Coordinate admissions and discharge activities
Monitor resident status and treatment plans
Complete accurate charting and documentation
Communicate with families and providers
Nice-to-have
Bachelor's degree in Nursing or Social Work
Registered Nurse (RN) license preferred
Two years clinical nursing experience
Certified Case Manager (CCM) certificate
Knowledge of Medicare and Medicaid programs
Key Requirements
Valid LVN/LPN or RN license
