• The Referral Services Specialist provides care coordination services evaluating options and services required to meet an individual's health care needs to promote quality outcomes.
• The Referral Services Specialists works collaboratively with members of the Keck Referral Center team to assess the needs of an individual and family, facilitate the development of an interdisciplinary plan of care, and coordinate care across service sites.
• Essential duties include reviewing incoming referrals, transcribing referrals into Ambulatory Referral Management, and collaborating with various departments to ensure smooth patient entry and processing.
